Wish List Application
FLY:FWD will accept Wish List applications on a rolling basis. Donations to eligible schools and organizations will be fulfilled based on available funding. Should your organization be selected, we will reach out to you directly.

You agree to our Privacy Policy and allow Operation Warm to provide your request publicly through our website, grant applications, social media, email and other communication channels we deem appropriate.

You agree that if you are selected to receive a donation from Operation Warm, you will complete our beneficiary agreement and understand that if you do not fulfill the requirements listed in the agreement, you may not be eligible to receive donations in the future.

You understand that you will be required to provide a valid tax ID (EIN) number for your 501c3 or school.

You understand that receiving a donation is not guaranteed. You agree that the below information about yourself, organization, and those served is true. You grant Operation Warm permission to use the information provided below as Operation Warm deems appropriate.
