1. Eligibility:  The FLY:FWD Sweepstakes (“Sweepstakes”) is open to legal U.S. residents  residing in Delaware, New Jersey and Pennsylvania, who are 18 years of age or older as of date of entry.  Employees and their immediate family members (parents, spouse, grandparents, siblings, children or grandchildren) and/or those living in the same household of Operation Warm (“Sponsor”), its advertising and promotion agencies involved with this Sweepstakes, including Don Jagoda Associates (“Administrator”), their parent companies, subsidiaries, divisions, affiliates and related companies, and the NFL Entities (as defined below)  (collectively, the “Sweepstakes Entities”) are not eligible to participate or win. All federal, state and local laws and regulations apply. Void outside Delaware, New Jersey, Pennsylvania and where prohibited by law. Sponsor’s decisions are final on all matters relating to this Sweepstakes.
    

2. To Enter: The Sweepstakes begins at 12:01 a.m. Eastern Time (“ET”) on November 7, 2024 and ends at 11:59 p.m. ET on November 7, 2024 (“Sweepstakes Period”). There are two (2) ways to enter the Sweepstakes: (1) You may make a minimum $16 donation online at INSERT.com and include all required information (“Donation Entry”). (2) If you do not wish to make a donation, you may enter by hand-printing your full name, full address, phone number, date of birth and email address on a plain 3" x 5" piece of paper and mailing it in a separate hand-addressed, stamped envelope with sufficient postage to be mailed to: FLY:FWD Sweepstakes, P.O. Box 7814, Melville, NY 11775-7814 (“Mail-in Entry”). Limit one (1) Mail-in Entry per envelope. Mail-in Entries must be complete to be eligible and must be postmarked on November 7, 2024 and received by November 12, 2024 to be eligible. Mechanically reproduced or automated Mail-in Entries are not eligible. As applicable, normal Internet/phone access and usage/data charges imposed by entrants’ online/mobile service will apply when entering via Donation Entry. All proceeds (Donation Entries) collected in this Sweepstakes, less the administrative fees, will be donated to Sponsor. Donation Entry and Mail-in Entry will be collectively referred to as “Entry” or “Entries”. Any Entry not in compliance with the above and which does not meet the other requirements of these Official Rules may be disqualified as determined by Sponsor in its sole discretion.
    

3.  Prizes, Grand Prize Details and Approximate Retail Value (“ARV”): Grand Prize (1): A Philadelphia Eagles package, including two (2) tickets to the Eagles vs. Steelers home game on December 15, 2024, two (2) Pre-Game Sideline passes and a $200 gift card for the Eagles ProShop. Transportation and accommodations are not included with prize. Tickets must be used on date specified or prize may be forfeited. Seat assignments are Sponsor-specified. If winner’s guest is a minor, it must be the winner’s child/charge. Winner and guest must abide by all venue policies and regulations, including without limitation all health and safety protocols, and ticket terms and conditions at all times. For the avoidance of doubt, unused tickets cannot be refunded, exchanged or applied towards tickets for any subsequent NFL game/event/exhibition or for cash. Sponsor, Administrator or the NFL Entities and/or venue operator reserve the right to remove or to deny entry to the winner or guest who engages in a non-sportsmanlike or disruptive manner or with intent to annoy, abuse, threaten, or harass any other person at the game or otherwise violates the applicable fan code of conduct. NFL game tickets awarded as the prize may not be resold, offered for resale, or used for any commercial or promotional purpose whatsoever. The ARV of the Grand Prize is $690. First Prize (1): A $300 Brooks Running Gift Card. Terms and conditions may apply to gift cards. No transfer, resale, cash or other substitution of a prize except Sponsor may substitute prize in whole or in part for one of comparable or greater value for any reason.  Limit one (1) prize per person/household.
    

4.  Random Drawing/Odds: Winners will be selected in a random drawing on or about November 15, 2024 by Administrator, an independent judging agency (whose decisions related to the drawing are final), from among all eligible Entries received. The odds of winning a prize depend upon the total number of eligible Entries received.
    

5.  Winner Notification/General Rules: All federal, state and local income taxes related to the total value of the prizes are the responsibility of the individual winners, as applicable. Potential Grand Prize winner will be required to sign and return a Declaration of Eligibility and Liability/Publicity Release within three (3) days of notification attempt, or prize will be forfeited, and an alternate winner selected, time permitting. First Prize winner will be required to verify address/eligibility within three (3) days of notification attempt or prize will be forfeited, and an alternate winner selected. Failure by the potential Grand Prize winner to provide complete and accurate information as required for tax purposes will result in forfeiture of prize. Return of prize/prize notification may result in disqualification and alternate selection, time permitting. Acceptance of prize constitutes permission to the Sweepstakes Entities to use winners’ names, photos and likenesses for promotional purposes without further compensation except where prohibited by law. Sweepstakes Entities assume no responsibility/liability for damages, losses or injury resulting from acceptance or use of a prize. Entrant/winner/guest irrevocably waive all claims against the National Football League, its member professional football clubs, NFL Ventures, Inc., NFL Ventures, L.P., NFL Properties LLC, NFL Enterprises LLC and each of their respective subsidiaries, affiliates, shareholders, officers, directors, agents, representatives and employees (collectively, the “NFL Entities”) and agree that the NFL Entities will have no liability or responsibility for any claim arising in connection with participation in this Sweepstakes or any prize awarded. The NFL Entities have not offered or sponsored this Sweepstakes in any way. No responsibility is assumed for lost, illegible, late or postage-due Entries. By entering, entrants accept and agree to be bound by these Official Rules and the decisions of the Sponsor/Administrator. Sweepstakes Entities assume no responsibility/liability for damages, losses or injury resulting from acceptance or use of a prize. Sponsor reserves the right to cancel, terminate or modify the Sweepstakes if it cannot be completed as planned for any reason, including but not limited to infection by computer virus, bugs, tampering, unauthorized intervention, technical failures, or corruption of any sort. In such event, Sponsor will award the prizes in a random drawing from non-suspect, eligible Donation Entries received prior to cancellation as outlined above, provided it is able to do so (along with all Mail-in Entries), and will announce the termination on the website.  
    

6.  Disputes: Except where prohibited, entrant agrees that: (1) all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prizes awarded shall be resolved individually, without resort to any form of class action, and solely and exclusively in a federal or state court located in Pennsylvania, and entrants consent to the mandatory and exclusive jurisdiction of such courts with respect to any such legal proceedings ; (2) all claims, judgments and awards will be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorneys' fees; and (3) under no circumstances will entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, indirect, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and all rights to have damages multiplied or otherwise increased. SOME JURISDICTIONS MAY NOT ALLOW THE LIMITATIONS OR EXCLUSIONS OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR EXCLUSION OF IMPLIED WARRANTIES, SO SOME OF THE ABOVE LIMITATIONS OR EXCLUSIONS MAY NOT APPLY. CHECK LOCAL LAWS FOR ANY RESTRICTIONS OR LIMITATIONS REGARDING THESE LIMITATIONS OR EXCLUSIONS. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Sweepstakes, will be governed by, and construed in accordance with, the laws of the commonwealth of Pennsylvania, without giving effect to any choice of law or conflict of law rules (whether of the commonwealth of Pennsylvania or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the commonwealth of Pennsylvania.
    

7.  Privacy: Information collected from entrants is subject to Sponsor’s Privacy Policy, located at https://www.operationwarm.org/.
    

8.  Winners: To receive a Winners List by email, send an email to: winners@dja.com with “Operation Warm Sweepstakes (01-2966-01)” as the subject line. Requests must be received by December 15, 2024. Winners List will be sent after prizes have been awarded.
    

9.  Sponsor: The Sponsor of this Sweepstakes is Operation Warm, 50 Applied Bank Boulevard, Glen Mills, PA 19342.
